Our Personalized Primary Healthcare experience begins once you have selected a doctor from a choice of highly-ranked physicians. The Nexus Healthcare team goes through an extensive validation process and works with physicians that not only have exemplary medical credentials, but also importantly have demonstrated a high level of patient care and service. Our care team then reaches out to schedule your Annual Comprehensive Executive Physical. At this personalized session, your physician and care team listens to any specific concerns that you have and conduct an extensive battery of testing and data collection to curate your Lifetime Electronic Health Record (LEHR™). From this detailed record of your health, your care team then works with you to create health and wellness plans tailored to your concerns and any issues identified during your onboarding process. **

By Your Side

Your care team stays by your side around the clock in two important ways. We are active participants in your personal health and wellness plans, and reach out at regular intervals to get your feedback and provide encouragement to support you in achieving your goals. Our Chiron™ wellness system also passively monitors your performance and health metrics through your personal health device to proactively alert your care team to potential issues that may be on the horizon. When any concerns arise, our team will reach out to schedule a consultation to determine any follow-on actions that are required.**

We provide 24/7 phone telemedicine consultations, as well as same day in-person/virtual appointments, where our care team will address any of your primary healthcare needs. With your Lifetime Electronic Health Record (LEHR™), immediate on-site testing, and our access to the latest clinical technology, we can provide a fast and accurate diagnosis to get you on the path to recovery on the same day of your appointment. If medication is necessary, our on-site pharmacy can immediately dispense many common and personalized medications. When a longer-term treatment plans is necessary, your care team provides a Personal Health Advocate (PHA) that creates a link between you as our patient, your Personal Primary Healthcare doctor, and your specialist network. Your PHA is a trained medical professional that has a single focus to drive the highest level of coordinated care possible across all of your healthcare providers. They communicate and advocate with doctors on your behalf, so that you are never alone in expressing your needs and exploring your best treatment options.**

When chronic health and wellness issues arise, your care team can also provide assistance with Chronic Care Coaches that specialize in supporting you proactively in the management of your specific chronic care experience.**
